Medical and Surgical Conditions

In order to meet the specific health needs of students, it is very important that the Health Office be informed of medical conditions/surgeries which may affect the student's school performance. Please complete the Disability Verification form for any medical accommodations. This should be signed by a doctor if there are specific accommodations that need to be managed.

If your student has a condition or upcoming surgery but there are no medical accommodations and you

All medical conditions such as seizures, severe allergies, diabetes, autoimmune disorders, heart or lung conditions or any condition that staff should be aware of should be entered into Healthy Roster at www.healthyroster.com in the Important Medical Information section under Student/Athlete Details and updated as needed.
