Medication Request Form

Students who require medication while at school will need to have the Diocese of Orange Medication Request Form completed and on file in the health office.  The form needs to be completed annually and is to be signed by the parent/legal guardian and the physician.  Students can bring the completed form and the medication in it's original container to the health office.

 

The medication will be stored in a locked medication cart in the health office. 

 

A separate form is required for each medication.

 

EpiPen's and inhalers can be carried with students however the medication form needs to be on file in the health office.
