Sports Physicals

 

All student athletes are required to have a current physical on file each year with Healthy Roster (to participate in MD summer camps and team sports).  

 

Our Sports Medicine Program is partnering with Exer Urgent Care to provide convenient and easy physicals for student athletes. With over 55 locations throughout Southern California, families can choose a convenient date and location between May 20 – August 31, 2024. 

A custom QR code is required to schedule your appointment. The cost is $40 per athlete and the physical is valid for a full year. Our Sports Medicine Program is vital to the health and safety of all student athletes. Proceeds from this partnership benefit the Mater Dei Sports Medicine Program providing supplies, equipment, continuing education for our athletic training team, and more.

1. CLICK HERE to register and purchase your QR Code (Note: the form will close Monday, May 20 at 9 a.m.)
2. QR codes will be released via email on Monday, May 20 to all registrants. 
3. Use the QR code to schedule your appointment at the location of your choice anytime between May 20 – August 31. (Health insurance documentation is not required.)
4. IMPORTANT: Print and bring blank MD physical forms to your child’s appointment (A parent/guardian must accompany the student athlete.) CLICK HERE to view/download from our Sports Medicine page on the MD Athletics website. 
5. After your physical, upload your completed physical forms to Healthy Roster on or before August 31, 2024.
