Physical/Screening forms due in Athletic Training Office prior to the start of academic school year. This is required for all incoming and returning Mater Dei Athletes. This date is specific for all athletes planning to try out or returning athletes who have not yet submitted a new physical. Any athlete not in compliance by this date will not be able to participate in any Fall athletic activity including sport specific classes or team weight training. Forms may be scanned and emailed to [email protected], faxed to 714-850-9539, or delivered to the Athletic Training Office (please make a copy for your records).
