Athletic Medical Physicals/screenings due for Summer Camp Participation

Physical/Screening forms due in Athletic Training Office prior to the start of any summer camp. This is required for all incoming and returning Mater Dei Athletes. This date is specific for Football, Baseball, Boy's Soccer, Girl's Soccer, Boy's Basketball, Girl's Basketball and Boy's Lacrosse. Forms may be scanned and emailed to [email protected], faxed to 714-850-9539, or delivered to the Athletic Training Office (please make a copy for your records).
