All returning and incoming athletes (including Cheer, Song and Dance team members) must have a completed Physical/Athletic Exam Screening on file at Mater Dei before they can participate in any summer and/or school-year activities.
The Physical/Athletic Exam Screening is valid for one year from the date it is completed and must be repeated on an annual basis.
For example, an exam screening completed on 6/21/09 will be valid through 6/20/10. Failure to renew the Physical/Athletic Exam Screening on/before the appropriate date will result in the student not being able to participate in any summer and/or school-year activities at Mater Dei. Participation will only be allowed after the exam screening has been renewed and officially received by Mater Dei Athletic Training personnel.
Mater Dei has concluded its on campus Physical/Athletic Exam Screenings that were coordinated for the 2010-2011 school year.
Physical/Athletic Exam Screenings must now be completed by your Primary Care Physician or alternative care facility (walk-in clinic, etc.). You may call the Athletic Training office at 714-754-7711 ext. 781 if you would like a referral to a physician.
