Thursday, June 6 ~ 9 a.m. - 12 p.m. (Includes EKG Screenings)
We recommend EKG screenings for members of the classes of 2010 & 2012

EKG screenings are being offered, free of charge by the Sparkling Angel Charities-The Kelly Weaver Memorial Fund.

We want all student-athletes to be able to receive the EKG screening, that is only possible if all forms are completed prior to arrival at Mater Dei.

You must bring in this form with you to the MDHS Athletic Screening.
If you choose to visit your own doctor you must bring this form to your doctor and submit it to Mike Fernandez in the Mater Dei High School Athletic Training Office. You may deliver, mail or fax your screening form.
INCOMING FRESHMEN ATHLETES: Please do not submit your forms before June 3rd.
