Young Adult Protection Workshop

What: Young Adult Protection Workshop
When: 6/19/2018, 6:00 PM – 8:00 PM
Where: LeVecke Center

Families of exiting seniors (who are already 18 years old) are invited to join us for this free and informative workshop.

Our evening will consist of an information session to review the process and documents that will be signed. Afterward, families will complete the fulfillment workshop with their child. They will sign and notarize: Advance health care directive, medical records release ("HIPAA waiver"), general power of attorney.

Students MUST be 18 on or before June 19 to complete this process and must have current government issued photo ID with them (Driver's License or State Issued Identification or Passport).
